Hangfires with my T/C Omega

When I shoot my T/C Omega I get good ignition of the charge for the first 2 shots and after that I get a hangfire. I am using the 209 primer with 2 777 pellets and a 250 grain sabot. It seems that the vent in the breech plug gets clogged because if I remove the breech and clean it I get 2 more crip ignitions. Any thoughts?
